Funny enough, this has happened at least three times with Time Warner Cable (now Spectrum)'s Kids On Demand feature:

Up into the early 2010s, they used their own on-demand preview station that played in the preview window. Somehow, in 2010 in North Carolina, someone at TWC managed to switch the preview channel with an adult preview channel, causing softcore nudity and explicit discussion from Playboy to play in the corner of the screen for a few hours one morning. Of course there were complaints and people who managed to record/take pictures (they're NSFW so I won't link them here...easy to find, though).

When Nick Jr.'s "NickMom" block first started, Nick Jr. was the channel that played in the preview window. This led to complaints to parents looking for kids' shows on demand who heard the raunchy bits of NickMom in the background. They switched the preview window station to Boomerang just a few weeks later. This is one I remember seeing.

Time Warner also used to split up their Movies On Demand channel onto a separate Kids & Family station - I'm not sure if they do anymore, but I saw a recording on YouTube of the preview window on that station playing an ad for a pay-per-view special called Hot Sxxxxy Mxxxx and Their Sex Toys. A little less than wholesome family entertainment.

To answer OP's question, I remember Old Spice being advertised during Adventure Time once, if that counts

I tried to find it on YouTube, but years ago, I saw a commericial Sheena Easton did for some heath club/spa that was kind of sexually suggestive, that aired in the Chicago area during DuckTales.

I couldn't find that particular one, but the ones I did find, well.....would you want them airing during the commercial breaks for SpongeBob, or something like that?

This commercial apparently aired on Cartoon Network in the mid-90s:

I doubt many parents were crazy about the Divinyls or R. Kelly clips, some might even have had a problem with the Right Said Fred clip.

Found it! (proof it aired during DuckTales is at the end):

This one is kinda mild, compared to the ones that played her song "Strut", but still. WHY?

One of the local ads I saw in a Cartoon Network Invaded commercial break during Camp Lazlo posted on YouTube was about Internet pornography addiction.

I also remember seeing trailers for The Hangover aired fairly often on Nickelodeon around 2009.
